Terms and Conditions – Oranga Hā incentives

Incentives after 4 weeks smokefree

Must be CO verified as a smoker (over 5ppm) to qualify for the programme.
May not be currently enrolled in any other incentive programme
May not have participated in any other incentive programme within the past 12 months.
Subject to eligibility and availability.
By taking part, you agree to the below criteria:
I must attend a regular clinic and be CO validated (under 5ppm) at the end of the programme to receive the voucher/s (sequence agreed upon with your SSP).
Any lost or stolen vouchers will not be replaced by the service. Please treat these like you would cash.
I can withdraw from the programme at any time but will not receive the voucher/s .
I can still be supported by the service if I withdraw from the programme.
My personal information will remain confidential and only anonymous data will be used in any reports on the programme.

Incentives for māmā hapū

You must be CO verified as smokefree to receive each incentive.
You can be supported by the Stop Smoking service at any time, but to be eligible for the incentives programme, these conditions apply.
To take part you must meet the below criteria:
I am pregnant and meet the programme requirements, and I would like to participate in this programme.
I understand that this involves weekly contact and CO monitoring for the first 8 weeks after my quit date, and then fortnightly contact and CO monitoring after that, until I complete the programme or withdraw.
I understand that I will only get vouchers if I do not smoke, as shown by the CO monitor – I need to blow under 5ppm.
I understand that my personal information will remain confidential and that only anonymous data will be used in any reports on the programme.
I understand that I can withdraw from the incentives programme at any time. If I withdraw from the incentives programme I can still be supported by the service if I want.

Incentives for supporters of māmā hapū

You must be CO verified as smokefree to receive each incentive.
You can be supported by our stop smoking service at any time, but to be eligible for the incentives programme you must be expecting a baby, or live under the same roof as a pregnant person.
Participants’ personal information is confidential to the programme provider. Any programme reports will include anonymous data only.
My partner is pregnant and enrolled in the programme, and I would like to participate for the benefit of me, my partner and our baby.
To take part you must meet the below criteria:
I understand that this involves weekly contact and CO monitoring for the 8 weeks after my quit date, and then monthly contact and CO monitoring after that.
I understand that I will only get vouchers if I do not smoke, as shown by the CO monitor – I need to blow under 5ppm.
I understand that my personal information will remain confidential and that only anonymous data will be used in any reports on the programme.
I understand that I can withdraw from the incentives programme at any time. If I withdraw from the incentives programme I can still be supported by the service if I want.

Incentives for whānau with young tamariki

You must be CO verified smokefree to receive each incentive.
You can be supported by our stop smoking service at any time, but to be eligible for the incentives programme you must have or live with young tamariki (under 5 years of age).
Participants’ personal information is confidential to the programme provider. Any programme reports will include anonymous data only.
To take part you must meet the below criteria:
I would like to participate for the benefit of me, and my whānau.
I understand that this involves weekly contact and CO monitoring for the first 8 weeks after my quit date, and then fortnightly contact and CO monitoring after that.
I understand that I will only get vouchers if I do not smoke, as shown by the CO monitor – I need to blow under 5ppm.
I understand that my personal information will remain confidential and that only anonymous data will be used in any reports on the programme.
I understand that I can withdraw from the incentives programme at any time. If I withdraw from the incentives programme I can still be supported by the service if I want.
